Currently we accept:

• Aetna

• OptumHealth Behavioral Solutions/United Behavioral Health

Clients will fill out the insurance information on the registration form that is provided.

Please Note: Aetna, and OptumHealth Behavioral Solutions/United Behavioral Health insurance companies need to be the client’s primary insurance in order for the insurance to pay for the services rendered. Medicare & Medicaid is not accepted.

Boerman Consulting LLC will submit the claim of each date of service on behalf of the client. Please Note: any portion of the bill not paid by insurance will be charged to the client’s credit card on file for the full payment of the services rendered.

Note: Pre-Licensed Professional Kaitlyn Hall does not accept any form of insurance.

It is important to note that if insurance is used, a diagnostic code is required & will be placed permanently on your medical records.

Co-pays are due at the time of the therapy appointment.
